Kimberly Williams is our in-house Medical Records/Disability Forms /FMLA Coordinator, who manages patient health records and serves as a liaison between patients, healthcare providers, and third parties like insurance companies, ensuring all forms are properly and accurately completed and submitted in a timely fashion that complies with legal and regulatory standards.

How do I request a copy of my medical records?

Is there a fee for obtaining my medical records?
Yes. For your full record there is a $25 processing fee. For a copy of your X-rays on a CD it is $10.
How long does it take to receive my medical records?
Once a request is made and payment is received, please allow 1-2 business days for processing and 1-2 days for processing X-Rays on CD.
Can someone else request my medical records on my behalf?
Yes. Only if that person is listed under your approved release list. If it is another doctor’s office, they will need to send our office a written request with a signed release from the patient.
Can I request only part of my medical record?
Yes. You can, however, the same fees as a full record may apply.
How long does it take to complete FMLA or disability paperwork?
For standard processing, please allow 5-7 business days; expedited forms will take 1-2 business days.
Is there a fee to have my disability or FMLA forms completed?

Can I email or fax my forms to the office?
Yes. you can do either. Or you can bring them in in person.
Can I request updates or extensions to my FMLA, or disability leave?
Yes. You can request an extension to be completed. The $15 additional form fee will apply, and the extension approval is at the discretion of your provider and approvals through your disability company are not guaranteed.
